NEW TOOL TO CHECK PRIOR AUTHORIZATION STATUS

Dear Providers,

We’re reaching out to let you know that RealRx, the Pharmacy Benefit Manager (PBM) for University of Utah Health Plans and Health Choice Utah, now offers a tool that allows you to check the status of prior authorization and formulary exception requests.

To access the status checker tool, please visit: realrxhealth.com/status.

Please have the following information available to complete the lookup:

  • Member name, ID, and date of birth
  • Requesting physician's NPI
  • Name of the drug
  • Date requested
